Fees & Insurance

Currently, the only insurance that Dr. Choi contracts with is Medicare. 

If you have a PPO plan from your insurance company, you may be eligible to receive partial or full out-of-network reimbursement for a neuropsychological evaluation. Dr. Choi is happy to provide you with the necessary information (typically a "superbill") to submit to your insurance to facilitate the reimbursement process.

Questions you can ask your insurance company:

1. Do I need a referral from my primary care doctor or specialist (e.g. neurologist, psychiatrist) prior to my neuropsychological evaluation?

2. What are my out-of-network benefits for a neuropsychological evaluation?

3. How much is my deductible?

4. Are there limits to the units of CPT codes? (See table below for codes)

5. What is the reimbursement process like? Where do I submit my claim?
